Lionheart Collaborative’s mission is to help individuals with disabilities and their families access essential, personalized resources so that they can thrive in their communities.

We provide person-centered planning and independent facilitator services, including transitioning current regional center clients into the Self-Determination Program (SDP) and modifying or maintaining their SDP plans year-over-year.
